Alterations in Regional Pulmonary blood flow and Hemodynamlcs


Abstract
Lung scanning provides clinically important information concerning many flow forms of congenital heart disease and regional distribution of PBF through the lungs influenced by posture, exercise, total pulmonary blood flow and changes in pulmonary artery pressure and pulmonary venous pressure.
In the present study the lung scan using conventional 99mTcFeMA and hemodynamic parameter in 14 cases of a various congenital heart disease (Table I & II) were observed to investigate the changes of pulmonary blood flow in the forms of obstruction of pulmonary outflow tract: a low pulmonary artery pressure and low pulmonary blood flow and increased pulmonary blood flow; elevated pulmonary artery pressure with left to right circulatory shunt, and to define the alterations of pulmonary blood flow before and after total correction of tetralogy of Fallot and subclavian to pulmonary artery anastomosis.
The upper and lower zone flows were low in patients with Tetralogy of Fallot and increased after total correction of-intracardiac anomalies.
Routine lung perfusion scan before and after operation for complex of pulmonary stenosis provides an early noninvasive means of assessment of its effect on pulmonary perfusion and it is important to know whether or not the systemic artery and pulmonary artery anastomosis is functoning properly.
Increased pulmonary blood flood with left to right circulatory shunt showed essentially similar perfusion in both lungs that of normal but increasing pulmonary artery pressure and elevated pulmonary vascular resistance had a higher flow through both upper lung zone but not more than lower, lung zone, and eventually Eisenmenger state with systemic trapping of radioactivity.
Patients with elevated pulmonary pressure & pulmonary vascular resistance of PDA had a shifting of blood flow to the oppsite site of lung with increasing radioactivity to the right lung and more on upper lung zone,
